In the field of long-term care, there is a widespread and critical issue that cannot be ignored: case managers, during home visits, are unable to record visit details in real time, and most still rely on paper records to complete these tasks. Not only does this make the entire process inefficient, but it also leads to potential errors or data loss. Imagine how devastating it could be if critical information is lost or incorrect when decisions are being made based on that data. That’s why we decided to develop an intelligent system for long-term care case managers to solve these issues — an online responsive home visit system that allows everyone to be much more efficient during home visits.
Core Motivation Behind the System
In the current long-term care workflow, case managers are responsible for conducting home visits, filling in visit records, and editing reports after the visit, which are then uploaded to the Ministry of Health and Welfare’s Care Service Management Information Platform. However, this process is still primarily paper-based, slow, and prone to errors. This not only slows down work efficiency but also impacts the quality of care. Our goal is to build a platform that allows real-time data updates and accelerates subsequent data uploads, reducing the workload of case managers and improving the accuracy of the data.
Two Key Features of the System
Real-Time Recording, Update Visit Data Anytime, Anywhere: Imagine being able to
record visit details during a home visit without flipping through paper records or waiting until
you're back at the office to organise the information. With just a phone or tablet, you can instantly
fill out the visit details, and all data will be saved and updated automatically. This not only
greatly improves work efficiency but also significantly increases the accuracy of the data. No more
worrying about missing information or making mistakes in your records — everything is instantly
reflected, ensuring a seamless workflow.
One-Click Report Generation, Easy Upload to the Ministry of Health and Welfare
Platform: Traditionally, after a home visit, case managers spend considerable time
reviewing and organising visit records before inputting the data into the Ministry’s platform. This is
time-consuming and can cause important details to be overlooked. However, our system automatically
converts the visit content into a report format, including case details such as the individual’s name,
gender, and physical or psychological needs. These reports can be downloaded with a single click and
directly uploaded to the platform, eliminating the need for redundant data entry and enhancing both
efficiency and data accuracy.

With this system, case managers are no longer burdened by the tedious manual data entry process. They can record home visit details anytime and anywhere, even while in the field, ensuring that all data is quickly and accurately saved. Moreover, the process of generating reports and uploading them to the platform is significantly shortened, allowing case managers to focus more time on the cases that truly need attention. This not only improves work efficiency but also stabilises the quality of care provided.
